Whether it’s a trip to the shops, a medical visit, or a social outing, we’re here to keep you active, independent, and doing what you love.

Please note that bookings are subject to driver availability. Please submit a booking request a minimum of 24 hours prior to proposed travel time.

Our experienced multi-disciplinary team offers a range of services, in-clinic and at-home. Our services are available privately or through the Commonwealth Home Support Programme (CHSP):
Thursday Transport Service
Patches operates a weekly hop-on, hop-off service each Thursday. The service operates back and forth between Toodyay-Northam-York. The cost is $20 per trip. This service stops at three locations including Toodyay Visitor Centre, Northam Visitor Centre, and York Community Centre. Please click here for timetable.
General Transport Services

HOW TO GET STARTED
To check your eligibility for supports under the Commonwealth Home Support Programme (CHSP), please contact My Aged Care online or call 1800 200 422.
They will check your eligibility and arrange for an assessor to visit you to determine the level of care you need. Private billing is also available.
